- We are not required to agree to your request
5. Get a list of those with whom we've shared your information
- Request an accounting of disclosures for up to six years
- One free list per year; may charge for additional requests
6. Get a copy of this privacy notice
- Request a paper copy at any time
7. Choose someone to act for you
- Legal guardian or medical power of attorney can exercise your rights
8. File a complaint
- If you believe your privacy rights have been violated
- We will not retaliate against you for filing a complaint
We are required to:
- Maintain the privacy and security of your PHI
- Notify you promptly if a breach occurs that may compromise your information
- Follow the duties and privacy practices described in this notice
- Give you a copy of this notice
- Not use or share your information other than as described here unless you tell us we can in writing
Changes to This Notice
We reserve the right to change this Notice. Any changes will apply to all PHI we maintain. If we make material changes, we will:
- Post the new Notice on our website
- Provide the new Notice during your next telehealth visit
- Make copies available upon request

We will not retaliate against you for filing a complaint.
Electronic Communication
We offer various ways to communicate with us electronically. While we have implemented reasonable security measures, there may be some level of risk in transmitting information electronically. By using our services, you acknowledge and accept these risks.
State Law
Where state law provides stronger protections for your health information than described in this Notice, we will follow state law.
